3 Essential Questions Covered Entities Should Ask When Considering an On-Site Pharmacy

April 21, 2025

Covered Entities face numerous challenges when operating a 340B Program, like manufacturer restrictions, changes in legislation, and more. One solution to these challenges is implementing an on-site pharmacy. But before you start thinking about a grand opening, we have 3 major questions that Covered Entities should be asking when considering an on-site pharmacy.

1. How will an on-site pharmacy benefit our patients?

On-Site pharmacies serve the community by filling prescriptions directly at the Covered Entity’s location, providing patients with easier access to medications, improving adherence and having a pharmacy team on location to answer questions.

Events like the grand openings and community events bring people into the pharmacy, helping you build relationships and create goodwill in the community. With a positive relationship, visitors will be more likely to return in the future, converting social capital into well-being for both the community and Covered Entity by supporting the pharmacy’s viability.

2. How will this benefit our organization?

Implementing an on-site pharmacy presents a unique opportunity to enhance operational efficiency and mitigate impacts of manufacturer blocks, two of the biggest challenges that Covered Entities face with their 340B Programs. On-site pharmacies enable your health center to potentially process more eligible in network prescriptions than those contracting with various outside pharmacies alone.

The streamlined operation of on-site pharmacy enhances patient care and satisfaction through real-time inventory management and immediate access to essential pharmaceuticals.

3. What are the potential challenges or barriers?

Implementing an on-site pharmacy is a complicated process including licensing, staffing, buildout of the pharmacy, inventory management, marketing, IT and more. Thus, hiring the individuals, with the right experience, needed to effectively run all the facets of a successful 340B on-site program can quickly become costly and time-consuming, unless you have the right partner.
